Our Purpose
The NDIS has the potential to transform lives — but only when it’s backed by providers who truly understand both the healthcare system and the human experience of disability. Rebalance Community Care was founded to bridge that gap.
We bring deep clinical experience into community care settings, ensuring participants receive support that is not only compassionate, but clinically informed, safe, and aligned with their individual goals.
Every decision we make is guided by a single question: “Is this the right thing for the participant?”
A community where every person with a disability has access to high-quality, consistent support — delivered by people who genuinely care about the outcome.
To deliver person-centred NDIS supports that are clinically grounded, responsive to individual needs, and focused on building participant independence and quality of life.
What We Stand For
Every participant is an individual. We build support around their goals, preferences and life — not around what's easiest to deliver.
We treat every participant, family member and referrer with the dignity and respect they deserve — always, without exception.
Our clinical background means we take safety seriously — robust risk assessment, clear incident reporting, and a culture of continuous improvement.
We work closely with participants, families, plan managers, coordinators and allied health professionals — because great outcomes require a great team.
We respond promptly to referrals, communicate proactively with all stakeholders, and make sure nobody is left waiting for answers.
We actively seek feedback, monitor our performance and invest in our team's development — because good enough is never good enough.
Our Expertise

How We Work
We start by genuinely understanding the participant — their goals, preferences, daily life and what matters most to them and their family.
Support plans are built collaboratively with participants, families and referrers — ensuring everyone is aligned before support begins.
We take care in matching participants with the right support worker — considering skills, experience, personality and practical compatibility.
Needs change. We check in regularly, review progress against goals and adapt support plans to ensure they remain relevant and effective.
